INSTRUCTIONS

1. Complete pages 2 and 3. On page 2, your sketch need not be a finished drawing. Appearance is not important at this stage as long as I can understand your invention. You will find it easier to explain your invention if you assign numbers to the parts.
2. In the written description of your invention on page 3, tell how it works, preferably by reference to part numbers on the sketch. Also, do include the advantages your invention possesses. Please print or write clearly.
3. If the space provided for drawings and description is not sufficient, use additional pages as necessary; however, be sure to have each of these additional pages signed and dated by your witnesses.
4. After completing the sketch and description (pages 2 and 3), have your witnesses complete the bottom section on page 1.
Do not use members of the family or relatives as witnesses.
5. Complete the top portion on Page 1, keeping the dates confidential.
6. While steps 1 - 4 are the preferable procedure, a notary public may be used as an alternative witness. Simply have the notary complete the certificate at the bottom of this page.
7. After completing this form, you should keep it in a safe place and it should not be shown to others. It is necessary to disclose the details of your invention to others for business purposes, photocopy pages 2 and 3 only. No one other than your attorney in a future legal action needs to know the dates or names of your witnesses on pages 1 or 4.
